Association between secondhand smoke exposure and mental health in Korean adults

Abstract
Objective: An association between secondhand smoking exposure and mental health has been studies in previous studies, but the association has not been fully investigated in the Korean general population. The aim of this study was to evaluate the association between secondhand smoking exposure and mental health problem in Korean never-smoking adults.

Methods: This study used data from the 2013 Korea National Health and Nutrition Examination Survey. In this cross-sectional study, 436 men (mean age 42.9) and 2,441 women (mean age 49.1) were included. To investigate the effect of secondhand smoking on mental health, we used survey logistic regression by sex. In this study, mental health problem was assessed by physician-diagnosed depression, depressive symptom, perceived stress, and suicidal ideation.

Results: After adjusting for demographic factors, lifestyles, and chronic diseases, comparing with nosecondhand smoking exposure group, secondhand smoking exposure group had more depressive symptom (odds ratio [OR], 1.79; 95% confidence interval [CI], 1.29 to 2.47; p <.001), perceived stress (OR, 1.74; 95% CI, 1.35 to 2.23; p<.001), and suicidal ideation (OR, 1.72; 95% CI, 1.11 to 2.65; p=0.01). However, physiciandiagnosed depression was not significantly different between two groups (OR, 1.11; 95% CI, 0.69 to 1.78; p=0.67). When stratifying by sex, women had strong associations between secondhand smoking exposure and mental health problems, but the associations were not significant in men.

Conclusion: Exposure to secondhand smokin was positively associated with depressive symptom, perceived stress, and suicidal ideation in Korean women. To decrease mental health problems, decision makers should strictly limited secondhand smoking and steady health promotion education related to antismoking is necessary.
